Please follow the links below to download the required form. Not all forms are applicable to you. Select the ones that are related to your need.

All travelers must fill these forms

• Visa appointment form - Click appropriate link to make your appointment online! - Abuja |  Lagos 

Please make sure that all the information is accurate, especially birth dates, names, e.t.c. to avoid errors in the visas as we scan all the information on the form as it is. Also make sure that you complete the final step that will give you the bar-coded form. You must not print if you cannot see the bar-code. The printed copies must be clear and dark.

Other forms you must provide in addition to the above mentioned forms:

• DS-157 form: All male applicants between the ages of 16 and 45 ONLY
• DS-158 form: All student applicants ONLY
